Geller; John J. <br />Flynn; Sharon MacKenzie <br />Health Department Attendees: Gerard F. Cody, Health Director; Ann Belliveau, Health <br />Clerk/Office Manager <br /> <br />Heiger-Bernays called the meeting to order at 7:00 p.m. <br /> <br />The Board reviewed the minutes of September 14, 2011, motioned to accept the minutes <br />as written with attachments. Flynn seconded. All approved. <br /> <br />Environmental Health Update/Reports - The Board reviewed the Environmental Health <br />Report prepared by Kathy Fox. <br /> <br />Food Inspection Program <br /> <br />Food Plan Review – Panera Bread – Cody introduced Stephen Harron, General Manager <br />for Panera Bread who appeared to request their food permit for the opening on November <br />28, 2011. After a discussion, Geller motioned that the Lexington Board of Health grants <br />an Annual Permit to operate a food establishment to Panera Bread located at 1684 <br />Massachusetts Avenue pending a successful pre-operation inspection. MacKenzie <br />seconded. All approved. <br /> <br />Food Plan Review – Dunkin Donuts at 915 Waltham St. – Cody introduced Bernard <br />Djevelakian to the Board of Health who was requesting a permit to operate a food service <br />establishment at 915 Waltham St. Mr. Djevelakian had recently completed a food plan <br />review with the Health Division. All required equipment had been installed according to <br />the code but a pre-operational inspection had not been completed. After a discussion, <br />Flynn motioned that the Lexington Board of Health grants an annual permit and to <br />operate a food establishment to Dunkin Donuts located at 915 Waltham Street pending a <br />successful pre-operation inspection. Geller seconded. All approved. <br /> <br />State Sanitary Code Update <br /> <br />16 Estabrook Road – Cody informed the Board of a recent letter sent to Elizabeth Weiss <br />on October 23, 2011. The Lexington Fire Department had responded to a call requesting <br />help to have the basement pumped out due to heavy rains and flooding. The Lexington <br />Firefighters observed conditions which make the house unsafe and unfit for human <br />habitation. Cody stated that heat had been restored to the residence but he would like <br />Mrs. Weiss to meet with Minuteman Services to see what services they may be able to <br />offer her. According to Cody, Mrs.Weiss has denied access to her home by Minuteman <br />Services. Cody will set up a meeting with Mrs. Weiss and Minuteman Services for <br />11:00 a.m., on Wednesday, November 16, 2011 in the Legion Room. <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />
