To Town of Lexington Please take notice, that on the evening of Friday, Aug 14. 1896 at about Eight o'clock, I was injured by being thrown from a carriage, by reason of the horse attached thereto falling into a trench across Mass. Ave., near the upper end of Hastings Park & about opposite the entrance to the grounds of the Boston Female Asylum- Said trench was open and neither protected nor lighted- I intend to claim damages for the injuries so received by me.
