A few people attending Discovery Day picked up the Alzheimer's Alert Information Sheet and
Registration Form.
Rose Goralnick and Jim Wanless went to the Fairlawn Nursing Home this afternoon (6/14) Rose
helped to complete the form where there were questions, and Jim took three (3) Polaroid
pictures of each client Ten (10) registrations occur-red -There are three (3) sets of
registration forms on each Alzheimer's client 1 remains at the C.-0.A , 1 at the Police Station„
and 1 at the Fire Station The four nursing homes, plus the Dana Home have all been contacted
via mail with a follow up call from Rose --
Susan Nordemo gave Alzheimer Alert Forms to the Visiting Nurses Association
`` Even though the second page of the June Newsletter gave a good rundown on Alzheimer's disease
\ and the registration of clients at nursing homes and at the Senior Center, as well as an
article in the Lexington Minute-man, perhaps the medical community could be made more aware of
this program
SENIOR HEALTH MONITOR CONTRACT OVERVIEW
Most likely a SHM contract will be signed by the Town of Lexington and VNCH There has been
-1-
considerable miscommunication and noncommunication on the part of the various parties invo red
When Linda meets with the Town Manager on 6/16 , they will discuss the SHM contract
Susan Nordemo wondered if some of the clients presently seen by VNCH wouldn't fall under the
SHM Program.

her Meals on Wheels position effective Friday, July 29th Evelyn has been at the C.O.A for

currently require a committment beyond twenty (20) hours per week

Board members to pass the word about these job openings, and have qualified persons submit (
their resumes to Suanne and/or Linda
