HUMAN SERVICES COMMITTEE

The meeting was called to order at 7 35 P.M by Carol Perez Since several
committee members were not present it was decided to not elect a new chairperson until
the next meeting, May 20 Nicky Osborne, vice-chairperson, will chair that meeting
Some discussion of the future meeting schedule took place
Mrs Perez reported on the Selectmen's meeting of May 12 at which the Reed

s�. ��I`he Town can request that information Mrs Perez presented
a copy of the process for siting community residences used by the Brockton area The
question of how the Town can work with private and private non-profit vendors was
raised What is the distribution of DMH funded residences in the entire Mystic Valley
area?
The HS committee would like the planning director to provide a map showing
existing subsidized housing community residences, nursing homes and the like The
concern of the committee is with the present distribution throughout the Town of
institutional and educational programs It was moved and seconded that the HS committee
having carefully weighed the concerns of the Reed Street neighborhood recommends the
following
1 that the Planning Board and the planning director should set up a map with
the location of all housing which could be defined as subsidized, community
residences and the like and that they develop guidelines for the equitable
distribution of such, to be sent to the HS committee for review
2 That the Selectmen should continue to consider guidelines such as the
Brockton model to inform the citizenry of short and long range plans for
such residences and agencies
The committee accepted the motion It was also thought that the Selectmen might want to
inform the Reed Street neighbors of these recommendations Nicky Osborne said that she
would like to know the reaction of the Planning Board to these recommendations After
that board had addressed the problem, perhaps the HS committee should meet with them
Finally, the question of whether the committee should be involved in the educational
process of the agencies (re community residences) was raised
A draft plan of goals and objectives was presented by Ms Osborne (see attached)
Some discussion followed Copies will be mailed to all committee members before the
next meeting
The agenda for May 20 was set as follows
1 Look at criteria again and firm them up
2 Review Ms Osborne 's draft of goals and objectives
3 Choose a new chairperson
Howard Levingston moved that the committee thank Carol Perez for her work as
chairperson It was so moved, seconded, and accepted Mrs Perez thanked the committee

for their cooperation with her
A short discussion of the elderly needs assessment followed Ruth Nablo
suggested that the committee look at the survey questions before they are used
The meeting was adjourned at 10 15 P.M

GOAL To insure the development and implementation of a Human Service System
which is responsive to identified needs and which promotes the well-
being and quality of life for all Lexington residents
OBJECTIVE To develop a two-year Human Services Plan for the Town of Lexington
Tasks 1) Review needs assessment
2) Conduct interviews to gather additional information.
3) Conduct an inventory of resources
4) Determine gaps in services
5) Develop a plan which prioritizes unmet needs
6) Review plan w/selectmen and community at large
OBJECTIVE To organize Human Service System which best facilitates the provision
of services provided by the Town
Tasks 1) Review current structure of how services are being delivered.
2) Determine strengths andiweaknesses of current system
3) R_ecgmm nd changes in structure or conf.rm existing structure
4) ` ° ew structure
OBJECTIVE To coordinate human services so as to avoid duplication and to in-
sure continuity of+care for clients served
Tasks 1) Determine referral patterns between agencies, institutions
2) Determine common goals among agencies
3) Determine natural linkages among agencies
4) Develop a structure (agreements, joint meetings, workshops, etc )
which promotes joint planning and coordinating efforts
OBJECTIVE. To evaluate human services program to determine how well goals and
objectives are being met
Tasks 1) Develop evaluation criteria
2) Apply evaluation criteria to programs funded by the Town
3) Make recommendations to the Selectmen on findings
