Sharon MacKenzie, R.N., CCM, served as meeting Chair Person. The Minutes of October 21, 2015 were approved with corrections. The Community Health report was reviewed and the following topics were discussed; Lexington Board of Health Minutes, November 18, 2015 Page 1 of 4 Community Health and Infectious Disease Report: Director Cody informed the Board on the various animal quarantine orders that were issued to prevent the possible transmission of rabies due to dogs or cats biting humans, pets, and wildlife or from wounds of unknown origin. Mr. Cody informed the Board about an upcoming health seminar. Across East Lexington, Don Rivard will be presenting to Lexington residents an educational pest management Seminar on December 9, 2015. Mr. Rivard will inform residents about what steps should be taken to avoid pests in your home short and long -term. Flu Clinic planning update: Director Cody presented the Board with the upcoming Flu Clinic Schedule. The Director also informed the Board of the total amount of Flu vaccinations given by age at the Lexington Public Clinics this year. As of this meeting 1,800 Flu vaccinations have been administered. The Environmental Health report was reviewed and the following topics were discussed; Environmental Health Report was reviewed for various inspections conducted by the Health Agent. Beijing represented by Ms. Xiao Feng Cherf, Manager and Ms. Maureen Lee of Food Service Solutions discussed progress and recent inspection reports. The Board spoke to the Beijing representative about the effects of no longer having a food consultant in their daily operations. State Sanitary Code Case Reviews: The Board reviewed the State Sanitary Code Case for 27 Vaille Avenue, 15 Diamond Road, 100 Adams Street, and 7 Graham Road. 27 Vaille Avenue; in the previous month, the presence of lead was found in the home, along with several other State Sanitary Code violations. The case is currently ongoing, and the landlord has been in contact with the Health Division to correct the violations in a timely manner. The tenant has recently raised concerns of a lack of effort by the landlord to correct violations. It is the tenants fear that these violations will be prolonged or possibly ignored. The landlord has offered to break the current lease in order for the tenant to move out, while work is underway. The said tenant has refused to accept this proposal by the landlord. 15 Diamond Road; the property owner submitted a request of extension to make repairs for the State Sanitary Code Violations. "After much discussion about the State Sanitary Code violations OTC dated October 9, 2015, the deadline to comply with the order was extended to Lexington Board of Health Minutes, November 18, 2015 Page 2 of 4 December 1, 2015." Sharon MacKenzie proposed the motion and John Flynn seconded the motion. All were in favor. 100 Adams Street; A state sanitary code inspection that took place at 100 Adams Street. Violations of the State Sanitary Code were observed. The property owner is requesting additional time to make clean up the debris pile and to keep the property in a clean and sanitary manner. The property owner submitted hand written request an extension to June 1, 2016. "After much discussion about the State Sanitary Code violations OTC dated October 27, 2015, I propose that the deadline to comply with the order be extended to December 31, 2015." John Flynn proposed the motion, David Gellar seconded. All were in favor. 7 Graham Road; A report was received by the Health Division from an abutter of 7 Graham Road. The report described a condition, indicating abandonment of the property. Upon follow -up inspection by this department, it was determined that the property had no running utilities and was currently vacant. A letter of Order to Correct was sent to the homeowner, without response. Mr. Cody informed the Board that recently the Health Division was made aware of an alternate address for the homeowner. A letter would delivered by a constable to this address later in the week. Tres Petite Creperie represented by Aristea Souliopoulos recently completed its food plan review with the Health Division. All necessary equipment, (hand sinks, prep sinks, hand wash sinks) have been installed except for the three bay sink. The representative came to seek a variance from the requirement of a three bay sink and to seek conditional approval to operate a food establishment. The Board asked operational questions about the new establishment. After some discussion: "The Lexington Board of Health grants an Annual Retail food Permit to Tres Petite Creperie upon successful completion of a pre - operational inspection, including all food plan review requirements." John Flynn proposed the motion, Sharon Mackenzie seconded. All were in favor. The Lexington Board of Health grants a variance to Tres Petite Creperie to substitute a mechanical warewasher from the requirement of Food Code 4- 301.12 Numbers and Capacities for Manual Warewashing, Sink Compartment Requirements. This variance will not require the permit holder to have the following piece of equipment: 4- 301.12 Manual Warewashing Sink Compartments. All Food contact equipment and utensils will be cleaned and sanitized in the Lexington Board of Health Minutes, November 18, 2015 Page 3 of 4 mechanical warewasher. The warewasher must meet the wash and sanitize temperature requirements of FC -4- 501.110 (2) and FC 4- 501.112 (A)(2) respectively. Sharon MacKenzie proposed the motion and David Gellar seconded. All in favor. The Emergency Preparedness report was reviewed and the following topics were discussed. The Board was updated on the upcoming Multi- Agency Avian Flu Working Group conference call on Monday, November 23rd. In an effort to facilitate the merger of Region 4A and Region 4B into one Health and Medical Coordinating Coalition, (HMCC), Lexington 4A MRC members and 4B MRC members will be part of a series of sessions in an effort to engage the five core disciplines. Sharon MacKenzie, R.N., CCM, motioned that the meeting be adjourned and John J. Flynn, J.D, seconded. All were in favor. The meeting adjourned at 8:57 PM.
