1) ALL CONSTRUCTION SHALL CONFORM TO THE MASSACHUSETTS HIGHWAY DEPARTMENT STANDARD
SPECIFICATIONS FOR HIGHWAY AND BRIDGES, 1988, AND THE TOWN OF LEXINGTON SUBDIVISION CONSTRUCTION
STANDARDS.
2.) ELEVATIONS ON THIS PLAN ARE BASED ON THE PLAN ENTITLED "JONAS CLARKE JUNIOR HIGH SCHOOL,
LEXINGTON MA, EXISTING CONDITIONS" DATED JULY 24, 1997, PREPARED BY HANCOCK SURVEY
ASSOCIATES, INC.
BENCHMARK: BOLT OVER , MAIN OUTLET OF FIRE HYDRANT WITH PUNCH MARK. BOLT ELEVATION= 223.22.
VERTICAL DATUM =N.G.V.D. 1929.
3.) RELOCATION OF UTILITY POLES AND UNDERGROUND UTILITIES ARE THE RESPONSIBILITY OF THE CUSTODIAN
OF THE POLE. THE CONTRACTOR SHALL BE RESPONSIBLE FOR COORDINATION ON UTILITY POLE RELOCATION.
4.) UTILITY LOCATIONS SHOWN ARE APPROXIMATE ONLY AND SHALL BE VERIFIED BY THE CONTRACTOR IN THE
FIELD. CALL "DIG SAFE 1 -888- 344 - 7233 ". THE CONTRACTOR SHALL COORDINATE ALL CONSTRUCTION
ACTIVITIES NEAR UTILITY LOCATIONS WITH THE RESPECTIVE UTILITY REPRESENTATIVE.
5.) THE CONTRACTOR IS RESPONSIBLE FOR ANY DISRUPTION OF THE EXISTING UTILITY SERVICE CONNECTIONS,
INCLUDING, BUT NOT LIMITED TO WATER, ELECTRICITY, TELEPHONE, GAS, AND CABLE TELEVISION, RESULTING
FROM THE CONTRACTORS WORK.
6.) THE CONTRACTOR SHALL PLACE 6" OF LOAM AND SEED IN DISTURBED AREAS WHEREVER PAVEMENT IS
NOT INDICATED.
7.) THE CONTRACTOR SHALL RESET SIGNS, MAIL BOXES, FENCES, STONEWALLS AND ANY OTHER EXISTING SITE
IMPROVEMENTS AS DIRECTED BY THE OWNER AND THE ENGINEER.
8.) THE CONTRACTOR SHALL ROUND SIDE SLOPES TO MEET EXISTING GROUND AT LOCATIONS DESIGNATED BY
THE ENGINEER. SLOPES SHALL NOT EXCEED 2' HORIZONTAL TO 1" VERTICAL (21 MAX.).
9.) ALL UTILITY COVERS WITHIN THE PROJECT LIMITS SHALL BE ADJUSTED TO THE PROPER GRADES., UTILITY
COVERS INCLUDE BUT ARE NOT LIMITED TO, CATCH BASIN FRAMES, AND GRATES, MANHOLE FRAMES AND
COVERS, GATE VALVES COVERS, CURB STRIPS AND HAND HOLES.
10) SUBJECT PROPERTY IS LOCATED WITHIN THE RS ZONING DISTRICT.
