GENERAL NOTES:
1) TOPOGRAPHIC INFORMATION OBTAINED FROM AN ON -THE- GROUND SURVEY COMPLETED BY COMMONWEALTH
ENGINEERING, INC. IN APRIL 2003.
2) WETLANDS DELINEATED BY SANFORD ECOLOGICAL SERVICES, INC.
3) THE PARCELS OF PROPERTY SHOWN HEREON LIE WITHIN ZONE C, AREAS OF MINIMAL FLOODING, AS SHOWN ON
THE "FLOOD INSURANCE RATE MAP" COMMUNITY PANEL NO. 250198 0005C, MAP REVISED SEPTEMBER 30, 1983.
4) CONSTRUCTION PERFORMED DURING THIS PROJECT SHALL CONFORM TO CONSTRUCTION STANDARDS OF THE
"LEXINGTON PLANNING BOARD'S LAND SUBDIVISION RULES AND REGULATIONS" IN PLACE AT THE TIME OF
APPROVAL, AND THE MASSACHUSETTS HIGHWAY DEPARTMENT STANDARD SPECIFICATIONS FOR HIGHWAY AND
BRIDGES, 1988.
5) UTILITY LOCATIONS SHOWN ARE BASED ON A FIELD SURVEY, CONTRACTORS NOTES AND RECORD
UTILITY PLANS. THE LOCATIONS OF UNDERGROUND PIPES AND CONDUITS HAVE BEEN DETERMINED
FROM THE AFOREMENTIONED ITEMS AND ARE APPROXIMATE ONLY. BEFORE PLANNING FUTURE
CONNECTIONS CALL "DIG SAFE 1 -800- 322 - 4844" IN ADDITION THE CONTRACTOR SHALL
COORDINATE HIS CONSTRUCTION ACTIVITIES NEAR UTILITY LOCATIONS WITH THE RESPECTIVE UTILITY
REPRESENTATIVE.
6) THE LOCATION OF THE EXISTING GAS AND WATER MAINS IS APPROXIMATE. THE DEPTH OF THE GAS
AND WATER MAINS AS SHOWN ON THE PROFILES AND CROSS SECTIONS IS ASSUMED TO BE 4' -5'. THE
CONTRACTOR SHALL TAKE ANY MEASURE NECESSARY TO ASSURE HIMSELF AS TO THE LOCATION AND
DEPTH OF THE WATER AND GAS MAINS AND ANY OTHER UTILITIES THAT MAY EXIST WITHIN THE PROJECT
LIMITS.
7) RELOCATION OF UTILITY POLES AND UNDERGROUND UTILITIES IS THE RESPONSIBILITY OF THE
CUSTODIAN OF THE POLE. THE CONTRACTOR SHALL BE RESPONSIBLE FOR COORDINATION OF UTILITY
POLE RELOCATION.
8) THE CONTRACTOR IS RESPONSIBLE FOR ANY DISRUPTION OF EXISTING UTILITY SERVICE
CONNECTIONS, INCLUDING WATER, ELECTRICITY, TELEPHONE, GAS AND CABLE TELEVISION,
RESULTING FROM THE CONTRACTORS WORK.
9) THE CONTRACTOR SHALL RESET SIGNS, MAIL BOXES, FENCES AND STONE WALLS AS DIRECTED BY
THE OWNER AND THE TOWN ENGINEER.
10) THE CONTRACTOR SHALL ROUND SIDE SLOPES TO MEET EXISTING GROUND AT LOCATIONS
DESIGNATED BY THE ENGINEER.
