Perlmutter; David S. <br />Geller; John J. Flynn <br /> <br />Health Department Attendees: Gerard F. Cody, Health Director; Ann Belliveau, Health <br />Clerk/Office Manager <br /> <br />Sharon MacKenzie called the meeting to order at 7:00 p.m. <br /> <br />The Board reviewed the minutes of March 9, 2011. Perlmutter motioned to accept the <br />minutes as written. Geller seconded. All approved. <br /> <br />Community Health Updates / Reports <br /> <br />Community Health Report – The reviewed the Community Health Report prepared by <br />David Neylon, R.N., and all follow-ups had been done in a timely manner. <br /> <br />Environmental Health Update/Reports <br /> <br />Environmental Health Report – The Board reviewed the Environmental Health Report <br />prepared by Kathy Fox. <br /> <br />State Sanitary Code Case Review – The Board reviewed the State Sanitary Code Case <br />Review. <br /> <br /> <br />a)Sizzling House of Kebab – New Establishment <br />Cody advised the Board that Farhad “Fred” Salemi, Owner of Sizzling House of <br />Kebab has completed a food plan review with the Health Division. All necessary <br />equipment has been installed according to code. Mr. Salemi appeared before the <br />Board to request a permit to operate a food establishment. After a discussion, <br />Flynn motioned that the Lexington Board of Health grants an annual permit to <br />operate a food establishment to Sizzling House of Kebab located at 321 Woburn <br />Street. Perlmutter seconded. All approved. <br /> <br /> <br />b)94 Hartwell Avenue – Waxy O’Connors Update <br />Cody informed the Board of a recent oil spill outside of Waxy O’Connors which <br />was discovered by Health Agent Fox during a routine inspection. Fox alerted <br />Karen Mullins, Conservation Director of the spill because of the environmentally <br />sensitive area surrounding this building who alerted Cody. Animal oil and <br />vegetable oil (fats, grease and non petroleum oils) are listed under hazardous <br />materials according to the Massachusetts Contingency Plan. Quantities of fifty- <br />five (55) gallons or more that are released or spilled into the environment are <br />required to be reported to the Mass Dept. of Environment Protection. Cody <br />prepared an order to correct letter. <br /> <br /> <br />c)Water Testing Project Update <br />st <br />Cody advised the Board that on April 1, he drove a BU School of Public Health <br />student and Wendy Heiger-Bernays to every school for an upcoming water testing <br />ndst <br />project to be conducted on April 2. This exercise on April 1 was to establish <br />where water samples would be drawn for lead testing. In November, the Fiske <br />School tested positive so the faucet was removed. At least four samples will be <br /> <br />
