Cody, Health Director; Ann Belliveau, Health <br />Clerk/Office Manager <br /> <br />Other Attendees: David Henley, Superintendent of East Middlesex Mosquito Control <br />Project <br /> <br /> <br /> <br />Heiger-Bernays called the meeting to order at 7:00 p.m. <br /> <br />The Board reviewed the minutes of August 11, 2010. MacKenzie motioned to accept the <br />minutes with corrections. Geller seconded. All approved. <br /> <br />Community Health Updates/Reports – The Board reviewed the Community Health <br />Report prepared by David Neylon, R.N., and all follow-ups had been done in a timely <br />manner. <br /> <br /> <br />Environmental Health Report <br /> <br />Mosquito Control Update –David Henley, Superintendent of East Middlesex Mosquito <br />Control Project updated the Board regarding what services Lexington receives as part of <br />The East Middlesex Control project. Henley also identified the single West Nile Virus <br />case in an older woman in Lexington. The mosquito control project was sampling for <br />mosquitoes. A discussion took place regarding the efficacy of spraying. <br /> <br />Septic System Renewal – 142 Marrett Rd. <br />Cody advised the Board that the Health Dept. had received the plans for the septic system <br />renewal in August but this was continued until the September meeting. Cody stated that <br />Meridian Associates provided all the necessary calculations, soil suitability assessments <br />(Form 11), percolation test (form 12) and they have also submitted detailed plot plans. <br />Health Agent Fox reviewed a checklist with Health Director Cody to ensure that all <br />necessary requirements of Title V have been met. In addition, comments have been <br />provided by the Lexington Conservation Division regarding this project. The plans as <br />submitted to the Health Dept. on August 18, 2010, may be approved with the following <br />conditions: <br /> <br />1.Provide a retaining wall as noted in Title IV, section 15.255. <br /> <br />2.Provide additional information about the impervious barrier. It is unclear <br />From the plans that it will be installed around the entire perimeter of the Soil <br />Absorption System (SAS) since it does appear to meet the minimum 15 ft. from <br />the side slopes. <br /> <br />3.Any activity within the 100 ft. butter zone to wetlands is subject to review with <br />Conservation Commission and will need at least a Request for Determination of <br />Applicability Filing with the Conservation Commission. <br /> <br />4.Clarify the use of the proposed “slaughterhouse and retail food” as it relates <br />the Septic System. <br /> <br /> <br />
