Lexington Board of Health Meeting, June 9, 2010 at 7:00 p.m. <br />Town Hall, Room G -15 <br />1 625 Massachusetts Avenue, Lexington, MA <br />Attendees: Burt Perlmutter, Chair; David S. Geller; Sharon MacKenzie <br />Health Department Attendees: Gerard F. Cody, Health Director; Ann Belliveau, Health <br />Clerk <br />Perlmutter called the meeting to order at 7:00 p.m. <br />The Board reviewed the minutes of May 19, 2010. Perlmutter motioned to accept the <br />minutes with corrections. MacKenzie seconded. All approved. <br />Community Health Report — The Board reviewed the Community health Report and all <br />follow -ups had been done in a timely manner. <br />Environmental Health Report <br />Cody informed the Board that testing at the Old Res will begin a few weeks before the <br />The Board reviewed the food inspection report. <br />shanghai -- Mr. Lee updated the Board concerning a recent inspection at Changhai. At <br />the inspection, three noncritical violations were found and no critical violations. After a <br />discussion, MacKenzie motioned that the Lexington Board of Health considers that <br />Changhai has completed all the requirements as discussed at the Administrative meeting <br />with the Health Director. Further Administrative action, including suspension and fines <br />will be imposed should similar or repeat food code violations (as noted in earlier reports) <br />continue to be observed at future inspections. Gellar seconded. All approved. <br />Formosa Taipei — Mr. Lee informed the Board of 8 critical violations at Formosa Taipei <br />during a recent inspection. After a discussion, Geller motioned that due to the number of <br />critical violations and repeat violations, the Lexington Board of Health is in agreement <br />with the corrective action plan as outlined at the Administrative meeting on May 3, 2010. <br />MacKenzie seconded. All approved. <br />The Board reviewed the food inspection report. <br />Housin LT dates <br />12 Calvin Street — Cody advised the Board that this residence has been boarded up by <br />Aftermath and the owner is now living in Waltham. All but one cat has been trapped. <br />16 Estabrook Road — Cody said the basement has been cleaned and working to get heat <br />installed by fall. Social Services has been working with the resident. <br />18 Tarbell Avenue -- Cody said this was a tenant based complaint with over 18 violations <br />identified. Cody spoke with the landlord yesterday but the tenant will not let the <br />contractors into the residence. A plan is being established to supply sufficient notice to <br />the tenants in order for contractors to enter the residence. The Fire Dept. was notified of <br />the fire code violation. <br />
