<br /> <br /> <br /> <br />Lexington Board of Health Meeting, May 19, 2010 at 7:00 p.m. <br />Town Hall, Room G-15 <br />1625 Massachusetts Avenue, Lexington, MA <br /> <br />Attendees: Wendy Heiger-Bernays, Chair; Burt Perlmutter; David S. Geller, <br />Sharon MacKenzie <br /> <br />Health Department Attendees: Gerard F. Cody, Health Director; Ann Belliveau, Health <br />Clerk <br /> <br />Heiger-Bernays called to order at 7:00 p.m. <br /> <br />The Board reviewed the minutes of April 14, 2010. Perlmutter motioned to accept the <br />minutes as written. MacKenzie seconded. All approved. <br /> <br />Cody introduced Larry Ramdin current President of MEHA and the Health Director at <br />the Town of Reading to the Board. Ramdin presented Cody with the President’s Award <br />for his support and had also been selected to receive Robert F. Periello Award for <br />Excellence in the field of Environmental Health for 2010. He also has been selected as <br />the sanitarian of the year. Ramdin also mentioned that Cody is the MEHA president elect <br />of effective July 1, 2010. <br /> <br />Community Health Report – The Board reviewed the Community Health Report and all <br />follow-ups had been done in a timely manner. <br /> <br />Environmental Health Report <br /> <br />Cody informed the Board that testing at the Old Res will begin a few weeks before the <br />opening to the public on June 5, 2010. <br /> <br />The Board reviewed the food inspection report. <br /> <br />Changhai – After a discussion, Perlmutter motioned that the consultant should continue <br />for one more month and then report back to the Board of Health. MacKenzie seconded. <br />All approved. <br /> <br />Formosa Taipei – Cody advised the Board that Formosa Taipei opened in February 2010, <br />and then at a routine inspection on April 28, 2010, Kathy Fox, Health Agent observed 8 <br />critical and 8 non critical food code violations. At an Administrative meeting with the <br />Health Director on May 3, 2010, the owner of Formosa Taipei agreed to the following <br />corrective actions: <br /> <br /> <br />1)The owner of Formosa Taipei must hire a food consultant to conduct 3 random <br />and unannounced State Santitary Code food inspections between May 15, 2010 <br />and August 15 2010. <br /> <br />2)The food consultant must review the file folder with the Health Director or Health <br />Agent before the first inspection. <br /> <br />3)The food consultant must be hired on or before May 15, 2010 and the signed <br />contract must be submitted to the Health Department. <br /> <br />4)The owner of Formosa Taipei must ensure that all staff complete the PIC training <br />held by the Health Dept. <br /> <br />5)The consultant must establish a cleaning schedule for Formosa Taipei <br /> <br />
