<br />Lexington Board of Health Meeting, November 10, 2008 at 7:00 p.m. <br />Cary Hall, Estabrook Hall <br />1605 Massachusetts Avenue, Lexington, MA <br /> <br />Attendees: Wendy Heiger-Bernays, Chair; Deborah Thompson; Sharon MacKenzie; Burt <br />Perlmutter <br /> <br />Health Department Attendee: Gerard F. Cody <br /> <br />Heiger-Bernays called the meeting to order at 7:00 p.m. <br /> <br />Minutes from the last meeting were not available. <br /> <br />Boston Sports Club – Camp Lex – Gerard Cody stated that representatives from Boston <br />Sports Club are before the Board due to serious issues regarding the permitting process <br />for Camp Lex who have been in Lexington since 2006. According to Cody, Ms. Fox has <br />been dealing with them and there has been repeated problems with obtaining complete <br />applications. Cody stated that during the past 2008 season, Camp Lex failed to provide <br />the following information to Ms. Fox at the Health Department: <br />Certificates of Inspection from the Building Dept. <br />?? <br />CORI and SORI reports for all staff (as of 7/30/08, only 8 CORI Reports <br />?? <br /> were provided) <br />Failed to provide immunization certificates for the camp staff <br />?? <br />Failed to provide immunization certificates for the campers <br />?? <br />Cody then gave Boston Sports Club an opportunity to speak to these issues and any other <br />issues relevant to this evenings meeting. Emily Colton of BSC stated that the only <br />documents on the list that was not provided to Ms. Fox before the end of the camp season <br />were the Cori and Sori reports. Ms. Colton said Fox stopped in the beginning of August <br />the Cori and Sori are not kept at the Club and must be requested from the HR department. <br />She requested them and then sent Fox an email saying she had all the results Cody <br />requested, that CORI, SORI and immumization certificate information be faxed to the <br />Health Dept. for any additional staff hired after the issuance of the permit. Cody also <br />requested that the Health Dept. receive a note indicating the number of new campers and <br />confirming proper immunization certificates. After a discussion, Perlmutter motioned <br />that Camp Lex/BSC submit all required applications, CORI checks, immunization <br />certificates any other required documents by June 15, 2009 to the Health Dept. and <br />Camp Lex/BSC will be denied an annual permit to operate a camp at this location if it <br />fails to submit a completed application and all other required documents by June 15, <br />2009. MacKenzie seconded. All approved. <br /> <br />Lemon Grass Update – Cody informed the Board that Lemon Grass will be presenting an <br />update this evening and perhaps after hearing what progress has been made, there could <br />be a consideration to close the case with a few stipulations. Maureen Lee, advised that <br />Lemon Grass has finished all of the on-site audits. Lee stated that the last audit found no <br />violations on her part and a recent Health Dept. inspection showed no violations. In <br />terms of the plan review and updating the facility, Lee said the landlord has not made a <br />decision concerning the property as to what space will be available. If the landlord has <br />not made a decision by December, an alternative plan would be to use the existing space <br />and bring it into compliance. Lee also stated that audits and on-site training will be <br />conducted every 60 days. Cody mentioned that the plan review is the only outstanding <br />issue from the original order. <br /> <br /> <br /> <br /> <br /> <br />
